Taking the first step: An assessment

If you are thinking of at home care for yourself or a loved one, the first step is an in-person meeting that we call an “assessment.”

First of all, it allows both you and our staff to discuss your situation and determine if in-home care might be a good solution.

Second, it helps us get to know each other. We know that hiring in-home care for a loved one can be a wrenching decision. It can be a stressful time. In-home care works best when there is genuine trust between our staff, our client and the client’s family.

We do not charge for assessments, and there is no obligation. Here’s what to expect:

Who does the assessment?

You’ll meet with one of our nurse care managers, each of whom is experienced in working with clients and families who need at home care.

Where will we meet?

Wherever is most convenient for you. It’s often helpful to meet at the home of the person who may need care, so we can evaluate the setting. But we are happy to meet you initially at another location, or you can come to one of our offices.

What will we talk about?

We want to hear about the situation that’s prompted you to contact us, and we’ll discuss the services that seem appropriate. Nurse care managers can explain exactly how our services work, what the cost would be and what other community resources might be helpful to you – such as delivery of meals, respite and day care options or relocation. We want you to have enough information to decided if in-home care is right for your situation.

What’s the next step?

If we both agree that in-home care would be appropriate and helpful, the care manager will draw up an individualized plan for care. The care plan will outline the types and level of care, expected family involvement and financial costs.
